You must fill out this Mandatory team roster before Friday, Aug. 21 by 5 PM. Lt. Gov. Kyle Erickson will need a team roster from each team playing this season. We have simplified this form so you only have to include players first and last name and if they have signed the waver and paid their fee only! (emails & phone numbers are optional)
Changes, additions or removal of players, to your roster can be submitted through our new Player Addition and Removal Form.
Please don't wait till the last minute to complete this form! Don't hesitate to contact Kyle Erickson, firstname.lastname@example.org, with questions about your rosters, players or league rules and requirements.
After Kyle has reviewed your roster for accuracy he will email you a copy for your records along with noting any corrections needed or request for clarification and player information.
